The Job Center offers targeted support to find job opportunities that match individual skills and health requirements. This includes:

  • Placement in suitable job vacancies that match health capabilities.

  • Support programs for further training or retraining to open up new career prospects.

  • Coaching to strengthen application strategies and personal skills.

  • Trying out new fields of activity to increase the chances of long-term employment.

Note: With the new basic income support, funding for such services is to be cut, which is why it is unclear to what extent the integration services will still be available.

Health promotion and prevention

Many job centers work closely with statutory health insurance providers to offer preventive health services. The aim is to identify or prevent health problems at an early stage. These include:

  • Health advice on nutrition, exercise, or stress management.

  • Courses and information events to strengthen physical and mental resilience.

Additional financial benefits: Additional needs in case of illness

Certain health conditions can lead to higher living costs—for example, due to special dietary requirements or medically necessary aids. In these cases, additional benefits can be applied for, which are paid on top of the citizens' allowance.

The amount and eligibility requirements depend on the individual situation.

  • With disability: 35% of your own standard needs

  • People with walking disabilities: 17% of your own standard needs

  • For expensive nutrition due to illness: 10-20% of your own standard needs

Sick pay and citizens' allowance

You will generally only receive sick pay from your health insurance fund if you are employed, have health insurance, and are unable to work (see below). If you receive citizens' allowance, you will generally not receive sick pay.

This may be different for people receiving supplementary benefits. If you are in employment that is subject to social insurance contributions, i.e., you are working and insured through your employer, you can receive sick pay if you fall ill. However, as this is often less than your wage, you can receive the difference from the JobCenter as a supplementary benefit. Therefore, be sure to notify the JobCenter if you receive sick pay and how much you receive.

Note: This only applies to jobs subject to social insurance contributions. As a mini-jobber, for example, you do not receive sick pay, but you may be able to receive the full amount of lost income from the JobCenter.

Earning capacity and basic income support

Whether and to what extent you can work depends on the degree of your health impairment. In principle, anyone who can work at least three hours a day under normal conditions and is not completely unable to work for more than six months is considered fit for work. In this case, you can receive citizens' allowance. However, if you are completely unable to work, i.e., you cannot work three hours a day, you can only receive basic income support due to reduced earning capacity.

To clarify these issues, job centers work together with three specialized services:

  • Medical Service (ÄD): Assesses how health restrictions affect career opportunities and recommends suitable rehabilitation measures if necessary.

  • Occupational psychology service (BPS): Provides support in identifying strengths, interests, and suitable activities, as well as in planning career goals.

  • Technical advisory service (TBD): Advises on how workplaces can be adapted to individual limitations in order to enable or secure employment.

Cooperation with external support services

Job centers work together with regional partners and specialist agencies to offer comprehensive support. This includes:

  • Psychosocial care: Help with mental health issues, family conflicts, or social problems. The aim is to create stability and develop new perspectives.

  • Addiction counseling: Support for alcohol, drug, gambling, or other addictions. Counseling can be combined with debt counseling services if financial problems exist.

Vocational rehabilitation

If you are no longer able to work in your previous occupation for health reasons, the citizens' allowance provides support for professional reorientation. This is done in cooperation with the relevant rehabilitation agencies, such as the Federal Employment Agency or the German Pension Insurance Fund.

Possible measures include:

  • Retraining or qualification programs.

  • Support in finding a job after completing the measure.

Procedures and evidence in the event of illness or disability

This is usually based on medical certificates, medical reports, or a severely disabled person's ID card, which document the degree of impairment and its effects on your ability to work. Notifications from other authorities, such as the German Pension Insurance or the Pension Office, can also serve as proof.

The job center reviews the documents and may consult the medical service or the occupational psychology service to obtain a professional assessment of your ability to work or your need for support. The documents must usually be submitted in the original or as a certified copy. Medical certificates should be as recent as possible to enable a reliable assessment.

In some cases, those affected are asked in writing to submit additional documents or to participate in medical examinations. This obligation to cooperate is crucial for benefits to be approved or continued. All medical data submitted is subject to social security confidentiality and will be treated confidentially.

Differences between incapacity for work and disability

In the context of citizens' allowance, it is important to distinguish between incapacity for work and disability, as these two terms have different implications for benefit entitlement.

Incapacity for work describes a temporary condition in which a person is unable to perform their professional activities due to illness or injury. This phase is usually limited in time and documented by a medical certificate (sick note). During incapacity for work, entitlement to citizens' allowance generally remains in place and sick pay is usually received if the person is in employment subject to social insurance contributions.
